Narrative

The Scan 360 Family Center assists families, who have a family member with a developmental disability to navigate the, sometimes very complicated and confusing service system. Family Navigators meet with families, develop a plan of action which addresses the family's unique needs , and assists the family in to trying to meet those needs. Connections to appropriate resources  for the family member (s) with disabilities is the primary focus, although other areas of need, such as housing are often  addressed as well. The program's outreach focuses on ethnically, linguistically and culturally diverse families, who are new to the area, have multiple unmet needs, and can be very  vulnerable. In addition, an enhanced information and referral service offers services to anyone who calls the center. The project has been designed as a “family-directed” initiative. Families have had  a direct role in the design, development and operation of the Center through participation on the Steering Committee which has at least 51% target family membership.
