Narrative

I am working in an established  private Home care  Nursing company  that offers services to the neighbouring  town of Thorndike, Warren, Brimfield, Mosom , Palmer, Wibraham,  Brimfield and Palmer.

 This company  will be a Limited Liability Company based in Palmer Massachusetts , owned by one operator with future hopes to grow and hire more staff.

Consumers of my services will be those individuals and families in need of home health care nursing  services with more focus to the geriatic and disabled population to keep them in the comfort of their homes , chronic disease management , transitional care,  case management ( professional oversight, coordinating care , patient advocate ( MD and specialist appointments)  , assessing and planning , medication management , wound care, and other nurse related services.
These patients are usually referred by other health care professionals such as physicians, attorneys, insurance companies and health care facilities 

With a good number of medical experience , hoping to serve this population to provide the best health care and keep patients safe in the comfort of their homes
