Middleton Family Medicine is your main source for excellent primary preventative care and multi-specialty care on the North Shore. We are committed to providing high quality healthcare with personalized attention that forms long-lasting relationships between our patients and our providers. This relationship will allow us to better understand you and your health. The health of you and your family is our first priority.

We have recognized the need for change in healthcare which is why we have created our office fundamentals, which is a set of values that our providers and staff live and work by on a daily basis. Our state-of-the-art medical office provides immediate effective care while still providing a familiar and  welcoming environment. We want to make health care services as easy to access as possible which is why we continue to grow our specialty services and diagnostic testing which currently includes laboratory services, ultrasound and x-ray.
