Narrative

To participate in this program one must 16 years or older, be eligible for MassHealth coverage and meet clinical criteria around assistance needed to complete certain activities of daily living such as bathing, eating, dressing, ambulating or toileting. A team consisting of a Registered Nurse and a Care Manager conduct the initial screening and after acceptance into the program will make ongoing home visits to provide nursing, case management and clinical supports. In addition to these supports there is also a financial stipend provided to the caregiver based on the level of assistance that the member requires.
