Narrative

Adult Family Care—also sometimes referred to as Adult Foster Care or AFC—is a program that supports families as they care for aging loved ones and family members with disabilities. Through AFC, you receive coordinated supports, resources, and funding that allow you to provide care at home.

We tailor our supports to each family’s unique needs and preferences. We honor and value cultural diversity. Our staff members come from a wide range of backgrounds and speak many languages, including Cantonese, English, French, Haitian-Creole, Lithuanian, Mandarin, Portuguese, Russian, and Spanish.

Your Loved One Must:

  • Be 16 or older
  • Be unable to live alone due to a physical, medical, or cognitive disability
  • Be eligible for MassHealth
  • Require help with one or more activities of daily living (ADLs)

We provide you and your loved one with:
 - Financial assistance
 - Regular visits from a registered nurse and a case manager
 - Resources and training
 - Referrals to services and community resources
