Narrative

The trained caregiver offers meals, personal care assistance, supports their activities in the community, and provides needed supervision.

Eligibility for AFC:

  • Is over the age of 16, and unable to live alone due to physical, emotional, or developmental disabilities.
  • Be MassHealth eligible (Standard or CommonHealth) with a doctor's written order.
  • Have a medical, physical, or cognitive condition that requires either hands-on or verbal daily assistance or supervision to complete one or more of the following activities:
    • bathing
    • dressing
    • toileting
    • transferring between positions
    • ambulating (standing and walking)
    • eating

Who can be an AFC Caregiver?

  • Parents, family members, friends - anyone 18 years or older. Cannot be a spouse or legal guardian
  • Must be willing to share a home with, and be trained to provide care for a person with a qualifying disability
  • Must be willing to follow a “Plan of Care” that will be monitored by AFC staff
  • Must complete an application process, including an interview, CORI check, medical examination and home study.
  • Must also participate in yearly trainings, complete daily caregiver logs and participate in monthly home visits.
