Narrative

HMEA’s Adult Family Care (“AFC” or Adult Foster Care) program provides ongoing support to individuals who cannot live alone safely and want to  remain in their current home and receive daily assistance from caregivers who are living with them.

HMEA provides ongoing professional and financial support to caregivers, often a family member, thereby preserving the relationship between individuals(participants) and their families. Our professional support includes case management and nursing oversight, including required monthly or bi-monthly in home visits.  Our financial support is competitive and based upon the needs of the individual requiring support. 

Participants in the AFC program must meet the following eligibility requirements as set forth by Mass Health:

  • Be eligible for MassHealth
  • Be at least 16 years of age or older
  • Unable to live on their own
  • Have a medical (or mental health) condition that requires daily, "hands-on", physical assistance or cueing and supervision during one or more of the following activities of daily living:
  • Eating - if the member requires constant supervision and cueing during the entire meal, or physical assistance with a portion or all of the meal.
  • Dressing - including street clothes and undergarments, but not solely help with shoes, socks, buttons, snaps, or zippers.
  • Bathing - full-body bath or shower;
  • Toileting - if the participant is incontinent or requires scheduled assistance or routine catheter or colostomy care;
  • Transferring - if the member must be assisted or lifted to another position;
  • Ambulating - if the member must be physically steadied, assisted, or guided one-to-one in ambulation, or is unable to self-propel a wheelchair appropriately without the assistance of another person

Each AFC Caregiver must meet the following standard qualifications as set forth by MassHealth:

  • Be a responsible person at least 18 years of age, with the ability to make mature and accurate judgments in the performance of the duties and responsibilities as a caregiver
  • Submit to a CORI check to determine if any offender records may disqualify the individual from becoming an AFC caregiver
  • Satisfactorily complete a physical examination and receive a tuberculosis screening in the past 12 month
  •  Be able to devote the appropriate time necessary to provide the needed personal care to the participant and ensure the participants safety and well being
  • Cannot be a spouse, parent of a minor child (including adoptive parent), or a legally responsible relative of the adult participant

 AFC Caregiver benefits include:

  • On-going nursing and case management support
  • Training and professional development
  • A Tax Free Monthly Stipend based upon the individuals level of need
  • On-going nursing and case management support

Responsibilities of the AFC Caregiver include:

  • Supervision and assistance with activities of daily living, and any other personal care that is necessary for the member’s well being
  • Monitoring and reporting changes in the participant’s medical condition
  • Maintain a clean, safe home with the participant with MA
  • Complete a caregiver log and submit at least monthly
  • Participate in monthly meetings with AFC Nurse and Care Manager
