Narrative

AFC enables the elderly and individuals with chronic illness or disabilities to continue living at home with their families, maintaining dignity and privacy.

AFC ensures that each participant receives the assistance they need with activities of daily living, such as personal care, meals, and supervision.

Our program provides training and support from a multi-disciplinary team including a registered nurse and a case manager.

Requirements for AFC Participation:

  • AFC is available to individuals over the age of 16 who cannot live safely on their own due to medical, physical, cognitive or mental disability.
  • Applicants must need supervision or assistance with at least a portion of their activities of daily living such as bathing, dressing, eating, toileting and/or ambulation.
  • Applicants must live in the same home as the caregiver.
  • Applicants must be eligible for MassHealth Standard or MassHealth CommonHealth. Applicants cannot also receive Personal Care Attendant (PCA) services or Home Health Aide (HHA) services.
