Narrative

Adult Family Care (AFC) lets the elderly and people with on-going illness or disabilities to keep living at home with their families. They can keep their dignity and privacy.

AFC ensures that each person gets the help they need with activities of daily living (ADLs), like personal care, meals, and supervision.

Our program provides training and support from a widely skilled team including a registered nurse and a case manager.

Requirements for AFC Participation:

  • AFC is available to people over the age of 16 who cannot live safely on their own due to medical, physical, cognitive or mental disability.
  • People must need supervision or help with at least part of their activities of daily living such as bathing, dressing, eating, toileting and/or walking.
  • People must live in the same home as the caregiver.
  • Person must be eligible for MassHealth Standard or MassHealth CommonHealth. They cannot also get Personal Care Attendant (PCA) services or Home Health Aide (HHA) services.
