Adult Family Care also known as AFC, is a program for individuals that is 16 years or older with developmental, intellectual, physical, or mental disabilities or elderly person who cannot safely live alone. AFC Members live with trained, paid caregivers who provide daily care.

The Adult Family Care program, provides individuals with personal care assistance daily in a family setting rather than a nursing home or other residential facility. Caregivers are to provide 3 healthy meals with 2 healthy snacks daily, companionship, personal care assistance and 24-hours supervision. Caregivers and the member must live together.

Our multidisciplinary team consist of a Director, Register Nurses, Case Manager, and Office Manager. Rivers of Hope staffs are well educated and trained to serve your family needs. We are culturally competent and culturally sensitive. Our team speaks Haitian Creole, English, Russian, and Lithuanian.
