Narrative

Adult Foster Care (AFC) is a MassHealth benefit that allows a provider agency, such as Waystone Health & Human Services to provide a daily stipend to a full-time caregiver of an individual with disabilities who is unable to safely live alone in the community, with a referral from the individual’s primary care doctor. Family members who are not legal guardians may become caregivers for an individual age 16 or older who have MassHealth Standard or Common Health insurance and due to medical, physical, cognitive, or psychiatric problems need help with their personal care.  Caregivers receive a tax exempt stipend, based on the member’s level of care, from Waystone Health & Human Services monthly to assist with the atypical expenses incurred due to the disabilities of the individual being served. The level of care for payment is determined by the Adult Foster Care Intake Nurse.

The AFC caregiver must complete a comprehensive application process, including a CORI check, medical examination, and home inspection. Caregivers provide assistance to individuals with activities of daily living such as; bathing, toileting, dressing, and other elements of personal care; house cleaning, laundry, and shopping. Individuals also are assisted with medication management, managing their health care, and are accompanied to their medical appointments.  Caregivers receive training, support, and respite time and participate in monthly home visits with the Adult Foster Care RN and care manager.
