Narrative

Adult Family/Foster Care is a Mass Health funded program that provides 24 hour services in a personalized home setting. This program allows individuals who require assistance with activities of daily living to maintain their dignity and pursue typical life routines with individualized assistance from a family or non-family caregiver. Eligible recipients of this program must be at least 16 years of age, receive Mass Health Insurance, either Standard of CommonHealth, and require assistance with completing tasks from a caregiver who lives in the same home. Adult Family/Foster Care (AFC) recipients must require assistance in at least one of the following areas: Mobility in or outside of their home, bathing, dressing, toileting, eating or transferring. A registered nurse and care manager will conduct an assessment to determine eligibility. The program is for individuals who need daily help with personal care/adl’s, but want to live in a family setting rather than in another facility. The caregiver provides meals, companionship, personal care assistance, and 24-hour supervision.

The Arc of the South Shore's program serves most of Southeastern MA but not Cape Cod.
