Narrative

Adult Foster Care provides care manager and RN support, as well as financial assistance, to qualifying families and caregivers to keep their loved ones at home and out of residential institutions. Eligible individuals must be at least 18 years of age, receive Standard MassHealth Insurance, and require assistance for any or all activities of daily living, such as dressing, bathing, toileting, mobility and eating; and/or 24 hour supervision.  The care provider can be a family member, such as a parent (who is not the Guardian), though Guardianship is not required.  Caregivers of individuals who are accepted into this program receive reimbursement determined by the individual’s level of need.
