Narrative

The Adult Foster (Family) Care Program supports individuals in the homes of their families or their own homes. The program is for individuals 16 years old plus with developmental disabilities or physical disabilities, and adults with physical disabilities, or mental illness. Through this program, the individual will receive the services and financial support they need while living at home with their families or companions/friends. The caregiver (who lives with the individual) will assist the individual with ADLs, and provide healthy meals, companionship, personal care assistance, and 24-hour supervision.​ With Rivers of Hope, you are not alone! We visit you monthly or more frequently if needed. Our multidisciplinary team consists of a Director, Register Nurses, Care Manager, and an Administrative Manager. Our team speaks English, Haitian Creole, and Spanish.
