Adult Foster Care

Massachusetts MENTOR offers AFC (Adult Foster Care) to adults with intellectual and developmental disabilities and elders with complex medical conditions who receive Mass Health Insurance, CommonHealth, or One Care MCO plans.  Our experienced team is available 24/7 to answer questions from the people we serve, their families, and their Mentors.

As with all our residential programs, adults in our AFC program are actively involved in planning their daily routine and the services they need to be successful. Our AFC program includes:

  • Life skills development

  • Opportunities to participate in community activities and day programs

  • Advocacy services

  • Community integration

  • Family involvement and support

  • Medication management oversight

  • Quality assurance

 

FAMILY FIRST AFC

In Family First AFC, a relative-who may already be the caregiver-becomes the individual's certified Mentor.  Whether the individual is a sibling with a developmental disability or an elderly parent with a complex medical condition, our supports help the individual stay with their loved ones in the family home.  Our expert staff provides individuals and their caregivers with a range of personalized supports, including in-home nursing consultation, personalized service planning, 24/7 on-call support, cand case management. 

It's All about Relationships

The Mentors who support our AFC programs come from all walks of life and are committed to making a difference.  We call them Mentor because they are more than care providers - they are teachers, advocates, family and friends who help the individuals we serve lead full lives in the community.  Our team of human services professionals and clinicians provide Mentors with a range of supports, including:

  • Support groups

  • Ongoing skill development

  • 24/7 on-call support
