Adult Foster Care (AFC) Our AFC program lets a family help or friend to provide 24-hour care in the comfort of their own home. This program supports eligible people who require assistance with daily living activities but prefer to remain in a familiar, family-centered space. Caregivers receive a tax-free stipend. Each participant benefits from personalized care coordination and ongoing clinical oversight. Group Adult Foster Care (GAFC) We also offer GAFC—a supportive care option for people in community housing or assisted living facilities. This program helps people maintain their independence while receiving daily help with Activities of Daily Living (ADLs) such as bathing, dressing, mobility, and meals. With regular nursing oversight and care management, we ensure each person's health, safety, and well-being are supported all the time.
