Narrative
Hospice is a specialized, patient-centered and coordinated model of medical care. It’s designed to optimize quality of life for seriously ill patients and their families, provided wherever the patient considers home, including private residences, hospitals, or long-term care facilities, anywhere in Suffolk, Norfolk, Plymouth or Bristol County.
Hospice care is:
- Patient-centered, doctor-directed and nurse-managed
- Focused on comfort, best possible quality of life, and coordination of care
- A medical model of care that meets the physical, emotional and spiritual needs of those living with life-limiting illness
- An interdisciplinary approach that includes a team of doctors, nurses, social workers, spiritual counselors, volunteers and therapists as appropriate
- Offered in homes, nursing homes, hospitals or hospice inpatient units
A program providing home care to terminally ill patients, and long term care facilities in their home. Offering palliative care, hospice focuses on the patient's comfort-physical, psycho-social, spiritual and social as the primary goal. In order to be a hospice patient there must be a limited life expectancy certified by a doctor, not limited to cancer & accepting all other diagnosis. Offers pediatric palliative consult, nursing homes, pastoral and volunteers.
