Narrative

Who is eligible?

An adult living with:

  1. A severe physical disability plus
  2. A secondary (sensory, cognitive, or emotional) disability which significantly affects the person's ability to manage their day-to-day life.

Potential applicants need to have:

  • Massachusetts residency
  • be able to handle the emotional stresses of community living with reasonable supports 
  • be their own guardian

Consumers receiving comparable services through another state agency or MassAbility Community Based Services (Waiver or SHIP) are not eligible.

The Adult Supported Living (SL) Program  provides case coordination to support people with organizing, finding resources and problem solving. Help is provided with:

  • finding accessible housing
  • management of the MassHealth PCA program
  • personal health
  • adaptive equipment
  • household
  • finanncial
  • social/recreational
  • vocational (work and job training)
  • transportation 
  • self advocacy

The program is rooted in the independent living philosophy. It empowers consumers to make their own choices about their lives. It is based on informed choice.
