Narrative
Who is eligible?
An adult living with:
- A severe physical disability
- A secondary (sensory, cognitive, or emotional) disability which significantly impede on the individual’s ability to manage their day-to-day life.
Potential applicants need to have a) Massachusetts residency, b) the ability to handle the emotional stresses of community living with reasonable supports and c) be their own guardian. Consumers receiving comparable services through another state agency or MRC Community Based Services (Waiver or SHIP) are not eligible.
The Adult Supported Living Program provides case coordination services to the individual not served by a similar state agency or program with a severe physical disability and an additional secondary disability to assist in managing and organizing day to day life activities. The case coordinator helps the consumer with organizing, finding resources and problem solving. Assistance provided with management of pca, personal health, adaptive equipment, household, finanncial, social/recreational, vocational, transportation and self advocacy. Individuals who are under guardianship are not eligible. The average level of case coordination is 4 hours per week.
