Narrative

We provide a variety of services to help elders and younger individuals with disabilities remain in their own homes. Our services are case management based. We are a Mass Health Personal Care Attendant Program provider.  We provide support services to Senior Care Organization and One Care Health Plan members. We work with TriValley Inc., Montachusett Home Care, Center for Living and Working and the Central Mass Area on Aging to administer the Central MA ADRC Option Counseling program.  The Family Caregiver Support Program  provides  education, support, and services to those caring for loved ones. Our LGBT Network provides education and support to elders and caregivers.  Our Care Transitions Coaching Program work collaboratively with local health care providers to support patients with complex needs as they transition home. For individuals with Medicare we provide health insurance information, counseling and assistance.  We encourage people to call us as we have an extensive, current database with resources for older adults. We are the agency to report allegations of Elder Abuse and Neglect. In addition to our listed service area we also serve the towns of Northborough, Marlborough, Southborough, Westborough and Hudson for the Protective Services Program.
