Narrative
PACT is a comprehensive service for individuals diagnosed with serious mental health conditions who may benefit from intensive coordinated services. PACT is based on the evidence-based national ACT model, and strives to maintain fidelity to that model.
Services are provided by an integrated, multidisciplinary team providing intensive and comprehensive community-based treatment and supports for individuals living in the community. Services are responsive to the changing needs of individuals served. PACT provides outreach, engagement, rehabilitation, clinical, health-related and recovery-based interventions and supports. The PACT team partners with persons receiving services to facilitate each person’s individual recovery journey.
