24-HOUR RESIDENTIAL SUPPORT SERVICES
Our 24-Hour Residential Support Services give people with brain injury a safe, comfortable home. They can be closely connected to family members, friends, and their community. People are helped by a team of support professionals. The team coordinates medical care, gives transportation to medical appointments, and helps with activities of daily living including meal planning and preparation and managing personal finances.
We work with people to foster social connections, friendships, and community membership. We respect and celebrate the person's personal choices, culture, talents, and contributions.
TRANSITIONAL ASSISTANCE
We provide Transitional Assistance for people with brain injury who are awaiting a move from nursing facilities or rehabilitative hospital settings to community-based Residential Support Services. They are assigned a Transitional Assistance support professional. That person helps make needed purchases and coordinates moving efforts. Meetings with the Transitional Assistance support professional are flexible according to the schedule and needs of the person being served. Transitional Assistance services are time limited and end once the person is settled in his or her new home.
INDIVIDUAL SUPPORTS
Vinfen offers highly-individualized support services to people with brain injury who are living in their own homes. They receive support from an Individual Support professional in the areas of:
- food shopping
- financial planning
- coordination of medical appointments
- activities of daily living (ADLs)
- meds management
- advocacy
- other areas
Supports are very flexible. Supports are tailored to the needs of the person supported.
PERSONAL CARE ATTENDANT SERVICES
We manage Personal Care Attendants (PCA) for people with brain injury living in their own homes. Vinfen acts on behalf of the person served to coordinate daily operations of PCA services such as:
- recruiting and hiring the PCA
- keeping person's information up-to-date with the PCA agency
- managing PCA hours
- changing PCA agencies when needed
TRANSPORTATION SERVICES
Vinfen offers transportation for people with brain injury who live in the community. They need help traveling in the community to work and other personal appointments. All transportation routes are previously approved by the funding source. Once a route is approved, the person can schedule transportation when needed.
