Narrative
The Medically Complex Program, known as Family Bridges, focuses on supporting families of children and young adults who are medically fragile. Following an initial home visit to complete a comprehensive needs assessment and determine eligibility for the program, the Family Bridges program will provide enhanced and specialized services using a family-centered approach. Supports may include:
- Service Navigation
- Funding for respite, uncovered medical goods
- Grant Writing
- Accessing programs/funding for home modification
- Accessing programs/funding to assist with adaptive vehicle purchase & modifications
- Educational and medical advocacy
- Information and Referral
