Narrative

F.O.R. Families, a joint program of the Massachusetts Department of Public Health and Department of Housing and Community Development (DHCD), is a home visiting program whose primary goal is to help families transition from homelessness into permanent housing. Home visiting services are available for families who are receiving Emergency Assistance (EA) from DHCD because they are experiencing homeless and living in shelter. F.O.R. Families Home Visitors are experienced social workers with extensive public health, community health and child welfare experience. They conduct family assessments and make referrals based on family needs. They help identify barriers to maintaining stable housing and work with families to address these challenges. Home Visitors coordinate services with an array of community-based programs and services such as WIC, Early Intervention, primary health care, domestic violence services, substance abuse and mental health treatment, the Department of Transitional Assistance, Housing Assistance workers, the Department of Children and Families and the Department of Mental Health when appropriate. They also provide information about school enrollment options, food, clothing and transportation resources.
