Narrative

Case Coordination Program provides support to families whose children have complicated health needs. It is a free, voluntary and confidential service. Case managers provide  consult to parents, educators, and medical and social service providers in home and agency settings. Also, information on available public benefits and community resources available. Case Managers can provide Family Support Funds for specific services. These funds enable families to purchase home care services for children during emergencies, on a short-term basis, or for limited respite care. Offices in Canton, Northampton, West Boylston, Tewksbury, Lakeville, and Jamaica Plain.
