Narrative
CCM Provides authorization and coordination of MassHealth Long Term Services and Supports (LTSS) which include Continuous skilled Nursing (CSN) services to a defined population of MassHealth eligible, medically complex members who require more than 2 hours of continuous skilled nursing services per day to remain safely at home.
