As the local Area Agency on Aging and the Aging Services Access Point, LifePath's foundation in serving older adults is clear. Programs that support them range from those designed to help keep you in your home. We can help you return there after a hospital stay. We can reduce your chances of readmission We offer Home Care Services. We have community living supports, such as Meals on Wheels and Congregate Housing. Dining Centers and Luncheon Clubs can help you become a more active part of your community. Other services available include:
