Narrative

Information and referral, case management, in home assessments, outreach consultation, community education presentations, caregiver support and support groups. Cost of fee depends on program. Partially funded by Older Americans Act/Executive Office of Elder Affairs.

As the local Area Agency on Aging and the Aging Services Access Point, LifePath's foundation in serving elders is clear. Programs that support elders range from those designed to help keep you in your home or help you return there after a hospital stay and reduce your chances of readmission, such as Home Care Services, to those that offer you support in community living situations, such as Meals on Wheels and Congregate Housing. Programs and services like Healthy Living and Dining Centers and Luncheon Clubs can help you become a more active part of your community. Other services available include support for grandparents raising grandchildren, help with benefits applications and managing your money, Medicare counsling through SHINE, and more.
