Narrative

The Community Supports Division at LIFE is divided into two discrete components: Individual Supports and Employment Services.  Services are provided to adults, 22 and older, who are seeking to learn to live and work on their own. The goal of the Community Supports Division is to help individuals with developmental and learning disabilities prepare for, obtain, and maintain their highest level of competitive employment, and to learn to live as independently as possible in the community of their choice.  LIFE has Supported Independent Living communities in Hyannis, Mashpee and Plymouth, and Group homes in Yarmouth, Centerville, Mashpee and E. Falmouth.  Each SIL community has Employment staff on site.

Program models include Individual Supports, Community Based Day Supports, and Individual Supported Employment.  CBDS offices are located at 24 Route 6A, Sandwich Ma, 02563.

LIFE offers Agency With Choice (AWC) services for applicants referred through the Department of Developmental Services (DDS). Based on the concepts of self-determination and self-direction, AWC empowers the individual and family to take an active role in the design, implementation, and evaluation of their services.
