Narrative

Day Habilitation Services for people with developmental disabilities (DD) are funded by MassHealth. Service plans are individualized based on each person’s service needs. Needs include:

• assessment and testing
• parent intake information
• professional evaluations in the areas of speech, occupational therapy (OT) and physical therapy (PT)
• the person’s current medical information

A Day Hab Service Plan is then developed by HOPe’s Interdisciplinary Team. Plan is done in accordance with MassHealth regulations. Individual goals and objectives are developed and targeted on the person's identified areas of need for skill acquisition, skill building, and greater independence.
