Narrative

The primary goal of the BFCH's Division for Special Health Care Needs is the promotion of high-quality, comprehensive, community based family centered systems of care for children with special health care needs and the provision of activities aimed at preventing secondary disability in individuals with special health care needs. Experienced Care Coordinators are located in regional health offices in each part of the state. Care coordinators help families obtain services, providing links to community- based resources and providing consultation to parents, educators and medical and social service providers. Care coordinators partner with families and providers and promote continuity of care. Bilingual and bicultural (in Spanish) Care Coordinators are available. Interpreter services are routinely accessed to support families.
