Narrative

Older adults are provided with customized care to meet their specific health needs. Our professional staff work with each person and their family to develop a person-centered care plan. We incorporate a myriad of community-based, day services, residential, and ancillary supports. Adult Companion: non-medical care, supervision, and socialization provided to assist with community activities, personal needs, and activities of daily living

Day Services: site-based and/or community program that assists with socialization, adaptive skills, plus a range of services including various therapies (physical, occupational, music, art, speech, and language), fitness and exercise groups, health, diet and nutrition groups, and social skill groups Individual Support and Community Habilitation: services and supports (one-on-one) necessary to ensure an individual’s independence, integration, and productive life in the community Memory Cafes: a welcoming socialization activity for individuals living with memory changes and their care partners to meet and share information and support.
