Narrative

Patients are cared for by a medical team including doctors, nurse practitioners, nurses, social workers and medical assistants.  Patients have access to specialists at Baystate Medical Center and Baystate Visiting Nurses Association and Hospice.  Home visits, diabetes care, nutrition counseling, diabetic foot clinic and financial counseling are a few of the resources offered.

Our geriatric medicine providers work with people over the age of 65.  We focus on the diagnosis, treatment and management of their often complex medical conditions.

Our team of doctors, doctors' assistants, nurses and social workers provide inpatient consultations and outpatient evaluations. We can help you and your loved ones prevent or manage the medical conditions that may accompany the aging process.
