Narrative

Patents are cared for by a medical team including physicians, nurse practitioners, nurses, social workers and medical assistants.  Patients have access to specialists at Baystate Medical Center and Baystate Visiting Nurses Association and Hospice.  Home visits, diabetes care, nutrition counseling, diabetic foot clinic and financial counseling are a few of the resources offered.

Our geriatric medicine providers work with people over the age of 65 and specialize in the diagnosis, treatment and management of their often complex medical conditions.

Our team of physicians, physician assistants, nurses and social workers provide inpatient consultations and outpatient evaluations to help you and your loved ones prevent or manage the medical conditions that may accompany the aging process.
