Narrative

Maestro – Connections Health Systems is an approved provider of the Group Adult Foster Care (GAFC) Program. This MassHealth-funded program enables eligible people to receive assistance with activities of daily living. Its goal is to help the residents of Massachusetts keep their maximum level of independence in their homes. To achieve this goal, support is provided by nurses, social workers, and personal care attendants. Eligibility Requirements:

• Must be 22 years or older Have or be eligible for MassHealth Standard Insurance
• Live in GAFC approved housing such as public or federal housing
• In need of assistance with one or more activities of daily living
• Have a Primary Care Physician (PCP) who believes his patient would benefit from GAFC services
