Narrative

We serve all of Massachusetts. 

Community Connection Healthcare(CCH) is an agency that provides Group Adult Foster Care (GAFC) & Adult Foster Care (AFC) programs for eligible seniors and adults with developmental disabilities. 

Our GAFC program service provides personal care and laundry services, grocery shopping, light cleaning and general housekeeping, medication reminder, 24/7 on-call access, supervision and assistance with daily activities on all individual care plans created by a Registered Nurse and Case Manager that is individualized for each client.  This care plan allows the client to be monitored for any changes in their medical status that are observed to be reported by the home health aide on a regular basis.  The client is also evaluated by either our Case Manager or Nurse every 30 days to ensure the client is receiving the proper care designed by the nurse and that they are provided with any necessary community resources such as transportation, food and any other government supported programs.  GAFC eligibility requirements for each client must be 22 years of age and older, live in subsidized housing, require at least one assistance with daily living and have MassHealth Standard or related Senior Care Options (SCO). 

Our AFC program provides supportive family living to elders and adults with disabilities that can no longer live alone safely.  The program is designed for those who would like to live in a family setting in their home rather that an institution.  AFC eligiblity for each client must be 16 years of age or older with disabilities who can no longer live alone, needs assistance with one or two assistance with daily living, must be MassHealth eligible standard or SCO, must have a medical diagnosis and need daily personal care, must have physician approval for program, must not be harmful to self or others, must be free of drugs and alcohol for one year, must be compliant with plan of care.  Caregiver can be relative, friend or non-related people with the exceptions of spouse, parent of a minor child or other legally responsible person such as a guardian.

Community Connection Healthcare (CCH) takes several measures to protect every consumer from any unprofessional acts each health aide staff's application is provided with a detail interview screening process covering CORI, SORI, OIG and personal reference checks, at the start of service the consumer is introduced with a meet and greet involving the Register Nurse, Case Manager and Home Health Aide to ensure continuity of care and to provide an accommodated schedule for all participants.
