Narrative
HOAP (Homeless Outreach & Advocacy Project) Housing manages scattered-site housing for formerly unhoused people and families. Clients in the HOAP Housing program have been given a housing subsidy through the Worcester County Continuum of Care (CoC). They receive ongoing supportive services. Case managers assist with many daily life activities like budgeting, appointment scheduling, transportation, and communication with property managers. They also provide referrals to healthcare providers as well as mental health and substance use treatment providers.
