Narrative

ABI/MFP Supervised Living provides services to adults living with Acquired Brain Injury (ABI) and other disabilities qualifying for the Money Follows the Person (MFP) waiver in Cape Abilities operated homes. Homes are located throughout Cape Cod. People with ABI are transitioned to live outside of skilled nursing facilities and other long-stay hospital settings. Our homes are designed and built to maximize accessibility and independence. People can enjoy the comforts and space of a private home setting.  Supports are needed to facilitate community integration, personal growth and enriched quality of life. All staff are provided with training specific to the unique needs of the people served.

People who qualify under the ABI/MFP Waiver programs

24 hours per day, 7 days per week.
