Narrative

Mental Health Clinic that provides a variety of services for mental health and substance abuse: counseling, medication clinic, day treatment program, partial hospitalization and dual diagnosis treatment for people with mental health and intellectual disability or substance abuse. BHN's Liberty Street Clinic provides mental health and dual diagnosis treatment and support to adults (17 and older). Programs below are funded by DMH, and/or most third-party payers. Liberty Street Clinic hours are Monday through Friday from 8:30 a.m. to 7:00 p.m. The following programs are offered through the Clinic: Medication Clinic: Psychiatric evaluations, as well as prescription and monitoring of psychotropic medications, are provided. The staff of RNs, clinical nurse specialists, and psychiatrists provide the following services: Comprehensive Psychiatric Evaluations Medication Monitoring and Maintenance Health/Medication Education Senior's Program This program, staffed by an RN, clinical nurse specialist, and a psychiatrist, specializes in serving individuals 55 years and older. The Senior's Program offers: Comprehensive Psychiatric Evaluations Medication Monitoring and Maintenance Consultation to Primary Care Physicians Health/Medication Education Family Support and Consultation Day Treatment Program This professionally staffed and managed program has various specialty tracks for individuals with the following problems: an acute mental illness, a serious and persistent mental illness, and dually diagnosed individuals with a mental illness and addiction. The program is up to 6 hours a day, 5 days a week, and provides: Individualized Treatment Stabilization of Acute Symptoms, Stress/Distress Management, Group Therapy, Prevention or Reduced Length of Psychiatric Hospitalization, Psycho-Education/Addiction Education Relapse Prevention Discharge, and Aftercare Planning Partial Hospitalization Services: to stabilize those at risk of psychiatric hospitalization. Services include: Individual Therapy, Group Therapy, Relapse Prevention, Pychoeducational Groups Outpatient Counseling Services include: Individual Counseling, Group Counseling, Couple/Family Counseling.
