Narrative

An eligible applicant must be between the ages of 5-25 and has sustained a traumatic injury through no fault of their own. 

To be eligible for an event you must have your child's primary care physician, or a specialist, submit a letter of recommendation stating why your son or daughter is eligible and a good fit for the program.

Please note that we ask all letters be submitted on an affiliated practice or hospital's letterhead.

We also encourage that you attach some of your son/daughters interests or favorite things to do as our goal is to provide long lasting memories. (ex: attend a sporting event, see a play, visit a museum, etc)

All applicants are selected by our memories committee.
