NaviCare (r) HMO's SNP is a Medicare Advantage Special Needs Plan and NaviCare SCO is a Senior Care Options program. NaviCare coordinates your health care needs to get you the most appropriate care. A team of doctors, nurses, social workers and other health care professionals work together to build a care plan that’s right for you.

NaviCare combines MassHealth and Medicare benefits for seniors 65 and older who live in Worcester County. They may live at home or in a NaviCare contracted nursing home or assisted living facility. A team of doctors, nurses, social workers and other health care professionals work together to build an individual care plan, designed specifically for your care needs. NaviCare includes 24/7 phone nurse access, full drug and over the counter meds with no co-payments, premiums or deductibles. The program includes home care, free medical transportation and more benefits.
