Narrative

Our PCHM progam provides personal caregivers and homemaking services for patients who cannot safely and independently perform activities of daily living (ADLs) and instrumental activities of daily living (IADLS) including but not limited to:

• Bathing and Dressing
• Feeding and toileting
• Transfer and ambulation
• Meal preparation and laundry
• Shopping and errands
• Medication reminders and cueing

We offer Adult Foster Care and Group Adult Foster Care services.

Our AFC programs provides personal caregivers and homemaking services for people who cannot safely and independently perform activities of daily living (ADLs) and instrumental activities of daily living (IADLS) including but not limited to:

• Bathing and/or dressing.
• Feeding and/or toileting.
• Transfer and/or ambulation.
• Meal preparation and/or laundry.
• Shopping and/or errands.
• Medication reminders.
