We are an inpatient skilled nursing facility with both geriatric and pediatric programs.  Our programs include specialties in children and adults with multiple disabilities

We are a pediatric skilled nursing care facility for profoundly, multiply disabled children providing respite and residential nursing care, rehabilitation services & education with specialized services.

Plymouth Rehabilitation & Health Care Center offers short-term rehabilitation, long-term, respite and hospice care. Our dedicated staff provides individualized care with compassion, dignity and respect. At our facility, you will find spacious resident rooms, a large and welcoming dining area and a comfortable and inviting living area.
