Narrative

Comprehensive rehabilitation facility committed to the treatment of patients in need of medical rehabilitation. Consists of both an outpatient and inpatient department. Criteria include physician's order, medical stability and rehab potential. Referrals accepted from acute care hospitals or self- referral. Interpret services available for Spanish, Polish, Italian, Vietnamese, French, Chinese. Home Evaluation Program provides a free in-home consultation with a rehabilitation professional who directs patients to the appropriate inpatient or outpatient rehabilitation services.

We do not take Connecticare, we do take certain Network Health plans and Senior Whole Health.  Call for more information.
