Narrative
RESIDENTIAL SERVICES provide a broad menu of living options and supports. Supports are determined by each person's personal preference and the level of care that is needed.
Our services include Community Residences, Independent Living, Adult Foster Care and Shared Living.
What we offer:
- Community Residences house people who need 24 hour support. Services are tailored to each person's needs to maximize their potential for independence and provide meaningful community connections. Services include intake and needs assessment, transition planning, individualized service planning, skills development and training, case management, recreation and community access.
- Independent Living assists those who have mastered many independent living skills. They still need enhanced support. Services include case management and support, assistance accessing community services, training, networking and recreation.
- Adult Foster Care provides clinical and financial assistance to qualifying families and caregivers. We help to keep their loved ones at home and out of residential institutions. Eligible people must be at least 16 years of age, receive MassHealth Insurance and require daily assistance with mobility, bathing, dressing, toileting, eating or transferring (ADLs). Caregivers of people who are accepted into this program receive payment determined by the person's level of need.
- Shared Living provides people the opportunity to become part of a family and a community. Shared living providers are carefully selected and trained to meet the needs and wishes of the people. Ongoing monitoring and evaluation includes monthly visits by the shared living case manager. We give access to the behavioral clinician, access to the residential nurse and crisis and safety management.
