Narrative

RESIDENTIAL SERVICES provide a broad array of living options and supports determined by each individual’s personal preference and the level of care that is needed. Our services include Community Residences, Independent Living, Adult Foster Care and Shared Living.

What we offer

  • Community Residences house individuals who need 24 hour support. Services are tailored to each resident’s needs to maximize their potential for independence and provide meaningful community connections. Services include intake and needs assessment, transition planning, individualized service planning, skills development and training, case management, recreation and community access.
  • Independent Living assists those who have mastered many independent living skills but still need enhanced support. Services include case management and support, assistance accessing community services, training, networking and recreation.
  • Adult Foster Care provides clinical and financial assistance to qualifying families and caregivers to keep their loved ones at home and out of residential institutions. Eligible individuals must be at least 16 years of age, receive MassHealth Insurance and require daily assistance with mobility, bathing, dressing, toileting, eating or transferring. Caregivers of individuals who are accepted into this program receive reimbursement determined by the individual’s level of need.
  • Shared Living provides individuals with the opportunity to become part of a family and a community. Shared living providers are carefully selected and trained to meet the needs and wishes of the individuals. Ongoing monitoring and evaluation includes monthly visits by the shared living case manager, access to the behavioral clinician, access to the residential nurse and crisis and safety management.
