Who is eligible for Adult Family Care in Massachusetts?
* Adults who are unable to live alone because of a medical, physical, cognitive, or mental disability
* Adults who require daily assistance with one or more activities of daily living (ADLs)
* Adults who are eligible for MassHealth

Mass Health funded program designed to allow a person, 16 years of age or older, with a medical or mental disability to live a meaningful life in a qualified home setting. AFC supports seniors and individuals with disabilities who are unable to live alone independently in the community. Participants may live with caregivers who are family members or non-relatives
