Shared Living

Our Shared Living programs allow individuals to become part of a host family, with all of the benefits and responsibilities that sharing a life and a household entail. The host family helps the individual connect with the wider community and provides the necessary personal assistance and training. Open Sky’s visiting nurses and service coordinators stand ready to provide expert assistance when necessary.

Adult Family Care

Adult Family Care (AFC) is a program that provides both financial support and practical assistance to caregivers. With a monthly stipend and personal, professional assistance from our staff, you can continue to care for your loved ones at home, where they are most comfortable. Plus, you and your entire family will have access to Open Sky's many resources and supports including social and recreational opportunities, behavioral health support, and more.
