Narrative

We are your town nurses providing caring and skilled services with a small town feel.The Stoughton Public Health Association is owned and operated by the Town of Stoughton.  We are a non-profit agency and have been Medicare Certified since 1966. 

Public Health Nurses provide essential community-based services focused on disease prevention and health promotion.  Examples of services provided include, but are not limited to:

• vaccine administration
• health screenings (blood pressure)
• surveillance of community infectious disease (TB, foodborne illness)
• offering health education
• conducting home visits for vulnerable members of the community

to improve health equity.
