Narrative

Teen Communities Living Program
CFP's spectrum of community living programs includes the Staffed Residential, Independent Living (IL), and Community Transitions programs. CFP provides youth in the Teen Commuity Living Program with the skills they need in order to be productive members of their communities when they leave our care.

The Teen Community Living Programs' goals are to help youth:

    * Develop real-life problem solving skills
    * Create permanent adult connections in the community
    * Achieve their educational aims
    * Prepare youth for employment and careers

Treatment Teams

Treatment teams serving clients in the Teen Community Living Program consist of clinicians, outreach workers, and milieu staff trained to assist clients struggling with mental health issues, substance abuse problems, and the effects of traumatic experiences. Treatment teams also work with teen parents, connecting them with day care, family supports, other parents, and parent education resources.


Pre-independent living

The Pre-Independent Living model is designed to assist young men and women, between the ages of 16 and 20, with addressing their emotional and developmental needs in a stable, supportive setting. This program coordinates therapeutic, educational, and vocational services as well as counseling services and crisis intervention. Youth are placed in staffed residences in the Boston metro region where they typically remain for up to one year. Youth in the Staffed Residential Program focus on expanding the skills they will need to live on their own. With the help of CFP staff members clients utilize community resources and develop long-lasting relationships. Youth in this program must agree to the following plans:

    * Be enrolled in an educational or vocational program part-time or full-time
    * AND Work part-time or full-time

Independent Living

The Independent Living model supports clients who have developed basic living skills but are not yet ready for complete independence. Youth live in unsupervised apartments, and CFP outreach workers meet with youth six to ten hours weekly. CFP subsidizes a portion of the rent. Youth continue to work or to go to academic or vocational school full-time, or to do both part-time. This model is open to youth between the ages of 17 and 22.


Community Transitions

This program is designed  to provide flexible supports for youth of any age, who live in any setting with a variety of service needs.  This highly individualized program assigns youth an Outreach worker who can assist youth in transition either returning home to adjust to family life, or leaving foster care for independence and needing additional supports.  The program seeks to provide supports for 3– 6 months, while focused on building long term and permanent supports for the youth.
